Each Cross Point member is asked to complete an Affirmation form by Wednesday, February 18. For each shepherd nominee, you may affirm the nominee, not affirm the nominee, or indicate that you do not know the nominee well enough to express an opinion.

• All forms, online and paper, should be signed by the member. Unsigned forms will not be considered.

• All forms will remain confidential within the committee.

• All forms should be submitted by Wednesday, February 18.
